This report outlines the investigation conducted by the Health Ombudsman into the adequacy of systems in place at Beaumont Care Aged Care Services concerning security of, and access to, schedule 4 and schedule 8 drugs.

Investigation report Beaudesert Hospital December 2015

This report details the investigation undertaken by the Health Ombudsman into the quality and safety of maternity services at the Beaudesert Hospital. The investigation commenced as a result of a complaint received by both the Health Ombudsman and the Metro South Hospital and Health Service (MSHHS) about a medical practitioner at Beaudesert Hospital. This complaint prompted a clinical review conducted by the MSHHS into concerns raised about the provision of maternity services at Beaudesert Hospital.

Investigation report Gold Coast Hospital and Health Service September 2015

The following relates to a complaint made in 2014 to the Health Quality and Complaints Commission (HQCC). The complaint was made by the daughter of a patient regarding the health service her late mother (the patient) received from the Gold Coast and Robina hospitals between 28 March 2013 and 25 June 2013. The patient died on 25 June 2013 while in the care of the Gold Coast Hospital.

Investigation report Townsville Hospital and Health Service November 2014

The following relates to a complaint made to the Health Quality and Complaints Commission (HQCC – Queensland’s health complaints agency from 2006 to 30 June 2014) in 2013 by an obstetrics patient who had a stillbirth at the Townsville Hospital Birth Centre (THBC).
