  1. The practitioner must practise only in employment and at practice locations approved by the Health Ombudsman.
  2. The practitioner must not be involved in any activity relating to the provision of food or fluid to a patient.
  3. The practitioner must not recline any patient with a swallowing difficulty, unless directed to by a registered health practitioner or in an emergency situation.
  4. Within five (5) business days of the commencement of these restrictions (if currently practising), and on commencing in any new place of practice, the practitioner must provide written confirmation to the Health Ombudsman that the practitioner’s employer/s and Director of Nursing (or equivalent) have been notified of the specific alleged conduct that gave rise to these restrictions.
