The practitioner has been issued with a permanent prohibition order by the Health Ombudsman, including that the practitioner:

1. must not provide any health service to females;.
2. must only practise as a health practitioner in employment and at practice location/s approved by the Health Ombudsman; and
3. must take all reasonable steps to ensure female patients are told at the time of wanting to book an appointment with the practitioner that they cannot.
