Please use the Health Service Complaint Form if you’re not satisfied with a health service provided by a health service provider, or you’re concerned with the health, conduct or performance of a registered or unregistered health practitioner.

We want to hear your feedback – good or bad – to help us ensure we are delivering quality services.

Provide as much information as possible to help us better respond to your feedback. There is no character limit. Please don't include private information like credit card details or medical history.
Please provide as much detail as you can (who, what, where, when and why) so we can pass on your compliment.

We can only assist journalists from recognised media outlets with media enquiries.

Please understand that we are governed by strict legislation to protect the privacy of those accessing our services. This means we cannot discuss individual complaints, complainants or health service providers. This includes any matters under investigation. We will provide you with as much information as we can, in line with our legislation.

This form can be used to make a complaint about our service, or the decisions or actions of the Office of the Health Ombudsman or our staff.
