The Health Ombudsman has written to all public and private health service facilities in Queensland, as well as the Queensland Ambulance Service, regarding changes to the reporting of clinical incidents to the Office of the Health Ombudsman.

Effective from 1 November 2015, reportable events reviewed using a method other than a root cause analysis (RCA) do not have to be sent to the office.
